Welcome to the second edition of our newsletter Risky Times.

Each issue will contain news and events relating to the out-of-hours and unscheduled/urgent care arena, together with examples of situations drawn from a Clinical Risk Assessment and malpractice claim reports.

We hope that you will find Risky Times useful and informative and we welcome any comments you might have on the content.

 

An unlucky break

Mr P was playing football with his sons when the ball struck his right hand, resulting in a hyperextension injury of the right wrist. He decided to deal with the resulting pain by applying an ice-pack and taking some analgesics.

Two days later the pain in his wrist had still not improved and he attended A&E at his local hospital. On arrival he was triaged to the walk-in centre adjacent to the unit where he was seen by an emergency nurse practitioner. On examination, a diagnosis of a wrist sprain was made, bandaging applied and Mr P discharged with instructions to mobilise the wrist.

The following week Mr P was still experiencing persistent pain and returned to the walk-in centre where he was seen by another emergency nurse practitioner. A possible scaphoid fracture was suspected and the nurse ordered a plain x-ray of the wrist.

She made no mention of a suspected scaphoid fracture on the x-ray request form.

Medical records and documents

MPS Educational Services carry out clinical risk assessments of out-of-hours providers and the following is an example of a typical scenario. Some of the facts have been changed to ensure confidentiality.

Part 1: Home consultation notes

This particular out-of-hours provider had a routine practice surrounding its doctors’ consultation notes made during home visits. This practice saw hand-written notes being later passed on to reception staff to be transcribed into the computer record; this approach represents poor practice.

Home visits by urgent care service doctors often involve very difficult clinical decisions. There is an unusually high risk of a subsequent allegation of inadequate care.

Complete and contemporaneous records are essential for the maintenance of good quality patient care and are needed if a claim is to be successfully defended.
